Lead form fields and messaging

Updated 2026-06-22Professional

This article covers the Form lead type — which fields it collects, the headline and description you can show, the submission ("thank you") message, and when the form appears relative to the results.

The lead form is set up in the editor's Lead tab. First make sure lead capture is enabled (the Lead switch at the top of the panel is on) and the Lead type toggle is set to Form. For the big picture, see Lead capture overview.

Fields

Under Form, switch on the fields you want to collect:

  • Name
  • Email

These are the fields available for the Lead-tab form. Both are on by default. Turn off any field you don't need.

Headline and description

Above the fields you can set the messaging the visitor sees:

  • Headline — the form's title. Defaults to "Get in Touch".
  • Description — optional supporting text below the headline. Leave it blank to hide it.

Button text

The call-to-action button that opens or submits the form uses the Button CTA text value from the top of the Lead panel (the Button group). It defaults to "Get in Touch". This is the label your visitors click to engage with the lead step.

When the form appears

Use the Show Popup dropdown to control timing:

  • Before submission — the form is shown before the visitor sees their results.
  • After submission — the form is shown after the results are revealed.

Pick After submission if you want people to see the value of the calculator first; pick Before submission if collecting the lead is the priority.

Submission message (thank you)

After the visitor submits the form, Numio shows a confirmation. Configure it in the Submission Message group:

  • Submission Message — the confirmation headline. Defaults to "Thank You".
  • Description — optional supporting text shown under the confirmation headline.

Where submissions go

Enter an address in the Send to field to have form submissions emailed to you. Each submission is also saved as a response you can review in analytics. For the full notification options — including workspace, sender, and custom destinations on the result-page call-to-action form — see Lead email notifications.
